When to use insoles?

I was wondering when people use insoles: immediately after getting new shoes (replacing ones that come in running shoes) or after the current insoles wear out? And if it's the latter, after running how many miles should they be replaced? Last thing: how long do custom insoles last (or does that depend on the runner?) Thanks!

  I have custom orthotics that I use for running only, they have over 3000 miles on them

  I never use them. I prefer as few moving parts as possible.
