Very satisfied with mine "hf5", bit in the higher price range but I never have the problem with them falling out.

I use the JVC Marshmallow In-Ear Headphones, and I love them! They are comfortable even for long runs of 20+ miles (the little earpieces are made out of memory foam and come with an extra pair in a smaller size). I have never had any problems with them slipping. They are DIRT CHEAP, too, and the sound is better than other pairs that I paid much more for. I actually bought mine at Walmart for about $10. (PS...I have no idea why, but while they come in different colors, the silver ones actually have a better Db range.)

I use the Jaybird Freedom JF3 bluetooth headphones. they are a little spendy at $99, but with their secure fit pieces, I can shake my head like a dog and they won't fall out. Sound quality is pretty good too for bluetooth, though I am no audiophile.

I use Bose in-ear headphones - they have special ear pieces that come with them that allow the headphone to stay more securely in the ear, the cord is extra long and the sound quality is excellent. They are a bit more pricey, but my last pair of Bose in ear just died after having them 3 years.

I have been using these and really like them! It's the sennheiser MX 680i.
I use them with my iPhone. I mention that because they have a remote on them so I can control the iPhone from the headphones, since I carry the iPhone in a pack that is clipped to my waist in the back. The headphone cord is long enough to allow me to do this, and they have a clip on the cord to keep the weight of the cord/control and my bouncing from pulling on the headphones. They also have "fins" to help keep them in my ears.
They have in-ear and earbud styles, but I suggest the earbud style since it will let in more ambient noise when you are running so you can be more aware of your surroundings.

I use Phillips over the ear/buds meaning they go in the ear and then there's a hook that goes around over the ear like glasses. I love them, they sound just fine and only about $10 at Wal-Mart. I don't feel so bad when they go bad from use and sweat. I hope this is helpful.

I use regular iPod earbuds BUT I found that if I run the cord down inside my shirt AND sports bra (to the side by my arm) my ear bud doesn't move around so much and stays in better. I also don't get so much noise from the cord moving all over the place. (I almost always run with only 1 earbud in so I just run it down the side where I have the earbud in and tuck the other ear bud into my sports bra for safe keeping!)
Maybe that can help until you find the right fit.

These were the ones I was looking at, but nervous that it was alot of money if they ended up still slipping out. Glad to hear you like yours.

I tried these first since they came with my iphone, but they're a little too big and end up hurting my ears after a while. :(

I use an over the ear style, single sided earphone, from onegoodearbud.com . Using the single sided earphone lets me be a little bit safer since it allows hearing what is around me as well as my music. I find the over the ear style doesn't slip.

As far as regular two earbud earphones...the same company sells something called BuDLoks, which are soft silicone earbud tips that you replace the original earbud tips with. These are similar in design to the Yurbuds shape, but with additional material that locks the earbud into the "bowl" of your ear. Each set comes with 3 sizes for custom fit. They work with a great number of in-ear style earbuds, such as those by Sony, Philips, and Skullcandy.
The best thing is these things are cheap:$12.95. Probably one of the less expensive options as you can take an existing headset and upgrade it.

Skullcandy Chops...the rubber they are made from is pretty sticky and they are very flexible. Being made from flexible rubber and not hard plastic makes for a more clingy and secure fit. I had problems with keeping most ear buds I tried in my ears when running (which I agree is very annoying and distracting) until I bought these. Hope this helps!

@VK - The jaybirds are a little spendy, but the sound quality is really good and they have lots of options to keep them in your ears. Also, they are guaranteed for the life of the headphones against sweat. I have even heard of folk who went swimming in them and had them go through the laundry wash and come out working perfectly. Of course, the mfg doesn't suggest doing either one of these. LOL.
